The True Story of the Aesthetic Breast

The first thing that comes to mind when it comes to breast aesthetics is the “artificial” appearance. But, why should the breast look artificial? Do silicone implants automatically give an artificiality to the breast? In my opinion, what needs to be evaluated is the characteristics of a natural breast.

 

Breast Behavior

Yes, totally true: Breasts have a behavior. Small or large, a natural breast behaves differently in different situations. Even though the breast tissue does not have a muscle or a joint to move the breast, it is indeed a moving organ! The breast moves while walking, running, waving, lying on the back, lying on the side, with or without bra. And it is this moveability that gives the natural appreance!   In conclusion, it is an immobile breast that looks artificial.

The breast tissue is a rounded organ that originates embryologically from the skin. There is no tight connection to the pectoral muscle or underlying rib cage. The breast rests on the rib cage easily, not tightly. Therefore, breasts behave more loosely than the chest and muscles during movement, and therefore swing. When the person is standing, the breasts sag lightly from the chest and assume the shape of a tear drop. When the person lies on her back, they take a round shape and slide sideways according to the incline of the rib cage. This is how natural breasts behave!

 

Breast Size

Some want big breasts. Some: small breasts. Some do not know what they want, while others can not express themselves correctly. Some know a great deal and come with a chosen implant model, and some have no idea and rely on the opinions of their spouses. Choosing prosthesis is almost the most important stage of breast augmentation surgery. What needs to be known is that every implant does not look the same on every patient. And not every breast size looks good on every woman. A breast size must be planned considering your height and shoulder and hip width.

Every woman has different chest width, diameter, and shape, different breast size, volume, shape and and different overall posture. The proportion of breast tissue relative to the rib cage, its position and laxity, the amount of skin, and the location of the nipple are all different. For this reason, the breast implant to be used in breast aesthetics should be specially designed for each patient. For a natural and beautiful result, breast implant should be selected according to the woman’s chest shape, breast shape, breast size and desired size. While volume of the implant is important, it is not the most significant parameter in considering your ideal breast.  My advice is that you imagine what your ideal breast looks like and try to articulate that.

 

Who understands? Who doesn’t?

Just as the personalized selection of the breast implant shape and size are important, the softness and the plan of placement of the implant are also among the important factors to be considered. An implant with similar softness to your breast tissue, when placed on the correct anatomical plane, is undetectable as a foreign entitity and ensures a desired natural effect.

 

Sub-Fascial Technique and New Generation Implants

In general, the implant is placed either under muscle (subcutaneously) or above muscle (superficially). The muscle here is the pectoralis major, which is responsible for the movement of the arm. Both the under- and above-muscle implant techniques have their own advantages and disadvantages. Placing an implant under the muscle is not actually a physiological or anatomic method of breast augmentation. After all, the natural breast tissue is not under the pectoral muscle. When placed above the muscle, on the other hand, the implant is visible or easily displaced.

Commonly known breast implant types are round and tear drop (anatomic) implants. The implants I describe as new generation implants change shape according to body movements. When the person is lying, these implants become round in shape, and while standing they assume a tear drop shape. Does this behavior remind you of something? Yes, the real breast actually behaves this way!

I use a technique called subfascial (under muscle membrane) technique in most of my practice. In this technique, the new generation breast implant is placed under the muscle membrane called “fascia,” not under the muscle itself. The fascia prevents implant displacement and there is no disadvantage of the subcutaneous technique because the muscle is not excised. The patient experiences almost no pain and, the most important thing is that since breast and implant are in the same pocket they act together. Because the new generation implant behaves similarly to a breast, the dynamics of the breast as a whole are very close to the a natural breast!
