Breast Aesthetics
The breast is a very important organ of every woman, and this increases the importance of breast aesthetics. The breast is an organ that has functional, aesthetic, and psychological roles. For a beautiful breast, the nipple should be in the right place and the size must be proportional to the body (shoulder, hip width) and rib cage. The consistency of the breast tissue and the stiffness of the breast are extremely important in terms of aesthetics. The most important thing is the dynamics of the breast. What I mean is as follows: although many people think of breast as a static organ, in my opinion, it is a highly mobile structure. When a woman walks, runs, lifts her arm, or lies on her side, the shape of her breast must change. For natural breast aesthetics, the breast that emerges as the end result of operation should act according to this dynamic.
Breast Augmentation Techniques
There are two options to add volume for breast tissue. One is fat grafting, which uses your own fat reserve, and two, silicone implants. Fat grafting is not usually appropriate for every patient. If the patient has small but anatomically appropriate breasts, and of course provided that there is enough fat tissue to be taken as a graft from the body, the breast can be enlarged within 1-2 session fat injections. However, if the skin is insufficient and the breast tissue is deformed, breast implant is essential.
Behind or in Front of the Muscle?
One of the most frequently asked and curious topics is the issue of placement of the implant behind or in front of the muscle. When we want to enlarge the breast, it is actually best to insert implant behind the breast. However, in very slender patients, or those with almost no breast tissue, the edge of an implant may be detectable through the skin and cause an artificial appearance. In slender patients therefore, in order to hide the upper half of an implant, part of the implant is placed behind the pectoral muscle, partially sacrificing functionality for the sake of appearance. The muscle mentioned (pectoralis major) is actually a chest muscle that controls turning of the arm.
What Is the SubFascial Technique?
Both behind muscle and behind breast techniques have their own advantages and disadvantages. Placing an implant behind a muscle is not actually a physiological or anatomic method of breast augmentation. After all, the breast tissue is not naturally behind the pectoral muscle. When placed in front of the muscle, on the other hand, the implant may be visible or displaced. This is why I prefer the subfascial technique rather than these two techniques in most of my practice. In this technique, rather than placing silicone implant behind the muscle, the implant is placed under a very thin hymen that covers the muscle, called the fascia. Whilst the fascia prevents silicone implant displacement, there is also no disadvantage of the behind-the-muscle techniqe since the muscle is not cut. The patient experiences almost no pain. But most importantly, due to the positioning of the breast and implant in the same pocket, the dynamics of the breast are very similar to a natural one.
Implant Shapes and Models
Commonly known breast implant types are round and tear drop (anatomic) implants. Which implant gives a more natural result varies from person to person. Implants I describe as “new generation” implants change shape according to the movement of the patient. Thus it carries features of both round and anatomical implants.
When is Breast Lift Needed?
In general, if the nipple is lower than where it should be or is larger than usual, a breast lift(mastopexy) may be necessary. If the breast tissue is sufficient, lifting breast by removing excess skin is sufficient, while in cases where breast tissue is insufficient, additional implants will be necssary (augmentation mastopexy).
Breast Reduction Techniques
Breast reduction is a very important operation, especially in young patients. Apart from the aesthetic end result of this operation, keeping breastfeeding ability and nipple feeling is important. The “central pedicle” technique, which is more difficult compared to other techniques, is my prefered techique in breast reduction/lift surgeries in suitable patients, due to very high breastfeeding rates, protection of the nipple sensation, and very natural aesthetic results.
Do Breast Reduction and Lift Surgeries Have Scars?
Breast reduction/lift surgeries have scars due to removal of excess skin. In general, the scars are in a “reverse-t” pattern. The quality of scar depends on factors such as precision of the surgical procedure, the use of cigarettes, the regular use of the medication prescribed, and genetic factors.

The price of breast augmentation, lifting, and reduction surgeries generally depend on factors such as the duration of surgery, the choice of implant brand and model, and the hospital where the surgery is to be performed.
