Eye Lid & Brow Lift
Eyelid aesthetics covers a wide range of procedures. Occasionally, a simple upper eyelid skin surgery may be sufficient. Sometimes a more complex eye, eyebrow, and cheek shape aesthetics may be necessary. What is important in eyelid aesthetics (oculoplastic surgery) is to correctly identify the patient’s eyelid problem and ageing components and plan correct surgeries to make the patient’s face beautiful without distorting the looks.
When planning the eyelid aesthetics, it is important to evaluate the shape, looseness, and excess skin of the upper and lower eyelids, eye angle (“tilt”, almond eye appeal), eyelid-cheek relation, and brow shape and position. Most people focus merely on one point and are unaware of other problems. However, for a natural result this area should be evaluated as a whole when listening to the patient’s expectation.
How Should Eyelid Aesthetics be Planned?
Upper and lower eyelid bags, excess skin and eyebrows are important factors affecting the surgical plan. An easy upper blepharoplasty is often enough for patients with only excess skin on the upper eyelid. However, middle and higher age patients often have eyebrow ptosis (lateral ptosis) in addition to lower and upper eyelid bags, many of which are unaware of this situation. In these cases, brow lift surgery should be planned in addition to eyelid surgery.
In some cases, a patient desires a more slanted eye shape (almond eye). Sometimes the patient is not aware, but has a low positioned mid-face. In this and similar situations, specific planning should be made, including modifying the midface, temple, and even eye angle. Thus, with a proper combination of surgeries, the patient becomes younger and more beautiful.
How are Eyelid Aesthetics Done?
It is possible to conduct aesthetic interventions on the eyelids and the surrounding mimic muscles. Botox can be applied to reduce the movements of the eyebrows, forehead, and muscles around the eyes and reduce wrinkles. Filler injections can be made for temporary correction of lower eye bags, if the deformity is slight. However, surgery is inevitable for a more permanent solution or greater deformities. Excess skin and bags under the upper and lower eyelids can be removed by surgical methods. Looseness of lower eyelid can be corrected, even eye shape can be changed if necessary (slanted almond eyes).
How is a Browlift Done?
While non-surgical methods can be used for browlift aesthetics, surgery is the right option for more permanent solutions. Needle radiofrequency (ScarletS) is one of the most effective methods of browlift without surgery (Ballerina Lift). However, this practice is more beneficial for younger patients. Generally, the excess skin needs to be removed in patients with lower eyebrows and skin laxity in the temple area (middle age and over). Various surgeries can be planned depending on the anatomy and eyebrow position of the person.
The inner and outer parts of the brow may be lifted together when a forehead lift if necessary. The outer part of brows is usually low and forehead skin is loose. The excess skin in this case is removed by surgery in the scalp or the hairline. While it is sometimes possible to lift eyebrows by direct removal of skin above eyebrow, sometimes eyebrow may also be repositioned above upper eyelid incision point.
What is No-Touch Blepharoplasty?
The surgery usually performed for the lower eyelid bags involves creating an incision at the base of eyelashes and removing the bags and excess skin from there. However, it is often possible to remove these bags directly from inside the eyelid. As there is no incision on the lower lid, the operation takes much shorter and the patient heals faster. Since the lower eyelid muscles are not cut off, eyelid loosening does not occur and the result is much more natural.
I often use the technique of transconjunctival blepharoplasty (removing eyelid bags without cutting the lower lid) in my facial aesthetic practice. I also use fat injection for fixing lid-cheek junction (tear-trough). I usually do the lower eyelid-cheek transition rejuvenation operation with this combination, often described as “no touch.” In this way I can fix lower eyelid bags and lid-cheek junctions without any scars in a much shorter and more effective surgery.
Is There an Age for Eyelid Aesthetics?
Whilst the need for eyelid aesthetic surgery is generally seen in middle age and older patients, this deformity is also sometimes seen at younger ages. Sometimes upper eyelids develop excess skin at an early age due to genetic reasons. Sometimes, upper eyelids swell and sag after pregnancy. At other times, young patients request that I change their eye shape or to raise eyebrows. Different operations can be planned for each situation.
Eyelid Aesthetic Surgery Recovery Process
Recovery time of eyelid aesthetic surgery is usually very short. When upper eyelid excess skin is removed, the sutures are removed in 3-5 days and the patient returns to normal life. In longer combined treatments, this healing process may be slightly longer. However, the edema usually passes within 1-2 weeks, and patient can easily return to normal life.
