Bichectomy
The most important factor in facial aesthetics that makes the person look young and attractive is the golden ratio. Speaking of the fullness of the face, along with the eyes, the eyebrows, the elasticity of the skin, the place and the shape of the chin, those slight dents in the cheeks and reverse triangle features hold a big significance. Our faces can have a fuller, “more cheeked” underlying structure that is genetic and independent of our weight. We can speak of a tissue of fat called Bichat fat pad or buccal fat, that lies in deeper layers of skin, that goes from the side of the mouth to the temples of the head. This anatomical structure is the most important fat tissue when it comes to the shape of the face. It is possible to remove some of this tissue with bichectomy procedure to make the cheeks slightly smaller and create those slight dents that make the face look more attractive.

How is Bichectomy Performed?
The Bichat fat pad is placed nearly in the deepest layer of the facial skin. You can reach this pad via facelift procedures that remove the SMAS flap (sub-SMAS). But when no face lifting is being performed, the easiest and safest way to reach this structure is through the inside of the mouth. The Bichat fat pad can be removed through a 0.5 cm incision during a 15-minute procedure, under local anesthesia. The patient can continue daily activities immediately.
What is the Recovery Period for Bichectomy and When Can You See the Results?
Applying cold compresses to the cheeks can reduce the risk of edema. Under normal circumstances, the patient can return to work and their daily activities the same day as this procedure. Because the incision is made inside the mouth, a liquid diet and the avoidance of hot foods is recommended.
Partial results can be observed immediately after the operation. However, final results will appear in six months.
Who are Good Candidates for Bichectomy?
Bichectomy procedures provide more striking results to younger people, between the ages of 20-35. The most suitable candidates for bichectomy are people with genetically full cheeks, independent of being overweight. The extension of the cheekbones and the fullness of the chin should be taken into consideration when performing this operation. It shouldn’t be forgotten that bichectomy is not an alternative to face lifting. I perform bichectomy in many of my face lifting and facial aesthetics operations to obtain correct facial proportions.
